Contributions of the Levator Ani Components to the Urethral Support Function in Women

چکیده

Hypothesis / aims of study Levator ani (LA) muscles which consist of the iliococcygeus, puborectalis, and pubococcygeus muscles are believed to play a significant role in the urethral support system [1]. However the relative importance of the three LA muscle components in contributing to the urethral support function remains unclear. The aim of this study is to specify the contributions of the three LA muscle components to the urethral support function in women using a subject-specific pelvic modelling approach.

Neural control of the female urethral and anal rhabdosphincters and pelvic floor muscles.

The urethral rhabdosphincter and pelvic floor muscles are important in maintenance of urinary continence and in preventing descent of pelvic organs [i.e., pelvic organ prolapse (POP)]. Despite its clinical importance and complexity, a comprehensive review of neural control of the rhabdosphincter and pelvic floor muscles is lacking. The present review places historical and recent basic science f...

Vaginal support as determined by levator ani defect status 6 weeks after primary surgery for pelvic organ prolapse.

OBJECTIVE To evaluate whether major levator ani muscle defects were associated with differences in postoperative vaginal support after primary surgery for pelvic organ prolapse (POP). METHODS A retrospective chart review of a subgroup of patients in the Organ Prolapse and Levator (OPAL) study. Of the 247 women recruited into OPAL, 107 underwent surgery for prolapse and were the cohort for the...

Quantification of levator ani cross-sectional area differences between women with and those without prolapse.

OBJECTIVE Compare levator ani cross-sectional area as a function of prolapse and muscle defect status. METHODS Thirty women with prolapse and 30 women with normal pelvic support were selected from an ongoing case-control study of prolapse. For each of the two groups, 10 women were selected from three categories of levator defect severity: none, minor, and major identified on supine magnetic r...

The Urethral Rhabdosphincter, Levator Ani Muscle, and Perineal Membrane: A Review

Detailed knowledge of the anatomy of the rhabdosphincter and adjacent tissues is mandatory during urologic surgery to ensure reliable oncologic and functional outcomes. To characterize the levator ani (LA) function for the urethral sphincter, we described connective tissue morphology between the LA and urethral rhabdosphincter. The interface tissue between the LA and rhabdosphincter area in mal...

Childbirth and the pelvic floor: ‘‘the gynaecological consequences’’

This review addresses the effects of childbirth on the pelvic floor, urinary continence mechanisms and the perineum. Genitourinary prolapse affects 15% of women and stress incontinence 20–30%. The major risk factors are age and childbirth, with severity increasing with parity. There are three mechanisms of support for the pelvic organs and bladder neck. These are (i) the muscular component: lev...
